The Russian monk’s gender-affirming journey and why Bengaluru is the favorite destination



Karma, a 29-year-old Russian monk, did not grow up with this name. He took this name when he decided to make a decision that would change his life. Karma landed in Bengaluru at the suggestion of a friend two months ago and underwent gender reassignment surgery at the KARO clinic in Bengaluru at Aveksha Hospital.

“I have always been alone. A friend from the transgender community suggested that I visit Bengaluru for the operation and that’s how I ended up here. I took my Buddhist vows at the end of 2018. As a child, I did not identify with a girl. I always wanted to be a boy. By the time I reached my teenage years, I had a clear picture of my sexuality by observing trans people around me, ”Karma told News18.

“I haven’t dated my parents yet. There are no such detailed procedures in Russia in a safe environment. So I opted for India. The financial problems also had to be resolved. My family lives in Russia, so I didn’t want to have the operation there. The center here also gave me a discount. My friend from the LGBTQ community told me it was affordable in Bangalore. It’s true, ”he added.

Happy with the decision he made, Karma said he now feels liberated. “I am freer than the average person. I will definitely continue to follow my vows and I am still learning my body. But I feel happy with the decision I made.

Destination Bangalore

The doctor who helped Karma through the change is Dr Roshan Shetty, plastic surgeon at the KARO Clinic at Aveksha Hospital in North Bengaluru. Its team is made up of a urologist, three plastic surgeons, a psychiatrist and an endocrinologist. Dr Shetty is undertaking gender reassignment surgeries with Dr Karthik Vishwanath, another plastic surgeon.

Karma heard about the doctor and the clinic through a friend and is one of many who recently traveled to Bengaluru to go for gender reassignment surgery.

Explaining the emergence of Bangalore as a preferred destination for surgeries, Dr Roshan Shetty said, “First and foremost is the quality of healthcare. Bengaluru is a favorite with people who want an array of specialty surgeries due to the quality of general health care. Indian doctors are preferred by more of the world because we treat 20 times more cases than the United States or Europe. Therefore, the skills are better.

“In addition, the specialists here are highly qualified and well trained abroad. The overall cost of health care is also much lower. It’s almost 1 / 5th to 1 / 10th the cost elsewhere. Medical tourism is also a factor here. People can do their procedures while on vacation. The demand is highest for cosmetic procedures, ”he added.

The trip

Gender reassignment surgeries, however, are different from others given the psychological weight of the decision.

“It’s not an easy process for anyone to walk into a clinic and have an operation. A very strict protocol is in place for international patients who wish to undergo gender reassignment surgery, ”said Dr Karthik Vishwanath.

Drawing up the protocol, he said, “First of all, the patient opting for surgery must undergo hormone therapy for six months. They should then change their name according to the chosen gender. This allows them to agree or deny their thoughts.

He added that patients should then seek psychiatric advice. “There are two psychiatric disorders associated with such cases that specialists diagnose. Gender dysphoria (a condition that describes a feeling of unease that a person may have due to a mismatch between their biological sex and gender identity) and body dysmorphic disorder (where one cannot help but consider themselves to be of the opposite sex). The psychiatrist will treat them accordingly and then issue a letter stating that the person is mentally ready and fit for the operation.

The patients then travel to the desired destination and an affidavit is written. Another psychiatric approval is recommended before having the operation. The patient must also travel on a medical visa.

Rhinoplasty, tummy tuck, breast augmentation, hair transplantation, and scar removal are the most common procedures that plastic surgeons must perform. But there is also a demand for breast removal and genital realignment in people opting for gender reassignment.
